My bmi is high, because I carry a lot of muscle and not because I am overweight. So If you have a lot of muscle you don't necessarily have to worry about your bmi.
Back in the 1950's about 4% of the food eaten was fast food. When this book was written, the number jumped all the way up to about 33%, so you can see why we are a fat society.
#1 If your insulin level is high, you will be fat... so avoid sugars.
#2 Do not mix fat and sugar together. They are both energy sources, so if you eat both there is no way you are going to burn off all of that energy.
